You are in the right place to learn the overall process to transition into the Self-Determination Program (SDP). Many find the process overwhelming due too much information at once.

To make it easier to understand, here the 10 main steps that align with the approach most California Regional Centers follow.

The transition has two key phases: 024 (PCP) and 099 (Pre-Enrollment).

  • Some Independent Facilitators (IFs) handle only the 024 phase, requiring a second IF for the 099 phase.

  • At OliveYou, we complete both phases, 024 and 099.

Self-Determination Transition Guide

Preparation: Most Common Inquiries

  • Qualifications: Open to permanent Regional Center clients.

  • Transition: Traditional services continue until the Self-Determination (SDP) officially begins.

  • Independent Facilitators (IFs):

    • Paid by the Regional Center during the transition to Self-Determination.

    • Paid from your budget for ongoing program administration. An optional line item in the Spending Plan.

    • Note: IFs do not replace the Service Coordinator. We are an addition to your team, working alongside the client and the Regional Center to ensure smooth transitions and effective program management.

  • Learn about the Self-Determination Program (SDP):

    • Talk to others who are familiar with SDP.

    • Discuss it with your Service Coordinator (SC).

  • Register for the SDP Orientation:


024 (Person Centered Plan)

Self-Determination Program Orientation

  • Required for all interested in Self-Determination

  • Attend the orientation (usually via Zoom).

  • Understand the program's goals and benefits.

  • Receive a certificate of completion.

Select Independent Facilitator (IF)

  • Research and select a facilitator who fits your needs.

  • Complete the facilitator’s agreement and onboarding forms.

  • Confirm their role in guiding you through the process.

Person Centered Plan (PCP)

  • IF schedules and facilitates the PCP interview (often via Zoom).

  • Discuss personal goals, hopes, and dreams.

  • Prepare and provide necessary documents (e.g., IPP, medical summaries).

4A Submit PCP to Service Coordinator (SC)

  • IF submits the PCP and supporting documents to your SC.

  • This step finalizes the 024-PCP portion of the transition.

4B Service Coordinator will reply with 2 requests

  • Sign the IPP/IPA agreement. This confirms that you've completed the Person-Centered Plan (service code 024) and approve initiating Pre-Enrollment Transition Supports (service code 099). This also authorizes the Regional Center to begin developing your budget.

  • Non-Medical Therapy, Social Recreation, and Camps

    • Activity & Provider Information for each

      • Activity, Provider Name, Email, Ph#, Address, Website

      • Registration (if applicable) + Monthly rate

      • Date Range

      • Miles from home

Pre-meeting with Independent Facilitator (IF)

  • Meet with your IF to prepare for the Budget Review Meeting.

  • Discuss potential services, including Non-Medical Therapy, Social Recreation, and Camps.

  • Clarify any questions or priorities to streamline the upcoming meeting.

Budget Review Meeting

  • IF coordinates with the SC to schedule the meeting (may require multiple attempts).

  • Submit the PCP for internal review (facilitated by the IF).

  • Sign the IPP/IPA Agreement to confirm the 024 phase is complete and transition to the 099 phase.

  • IF supports you in reviewing and negotiating the draft budget.

  • Note: This step can be time-consuming due to scheduling and detailed reviews.

Spending Plan Creation

  • IF works with you to develop a plan for services based on the approved budget.

  • IF helps prioritize services to meet your goals effectively.

  • Calculate mileage with guidance from the IF.

  • Note: This step requires careful planning and attention to detail.

Selection of FMS

  • IF provides options and explains FMS models: Bill Payer, Co-Employer, Sole Employer.

  • Compare available FMS providers based on costs, services, and reputation (guided by IF).

  • IF supports finalizing the selection and onboarding process.

  • Note: This step can take extra time to ensure a good fit.

IPP Self-Determination Assessment

  • IF assists in preparing for the IPP assessment with the SC at your home.

  • Confirm all documents and plans are ready for review.

  • IF may participate via Zoom if necessary.

Start Self-Determination

  • Begin on the 1st of the following month.

  • Transition from traditional services to the Self-Determination Program.

The Self-Determination Program (SDP) is an initiative in California that gives individuals with disabilities and their families more control and flexibility over their services. It’s based on five principles: Freedom, Authority, Support, Responsibility, and Confirmation. With SDP, you choose the services that best fit your needs.

How do I start the Self-Determination Program?

To start the Self-Determination Program, you need to attend an orientation. You can sign up for either the DDS & SCDD Statewide Self-Determination Orientation or your local Regional Center’s orientation. Once you’ve completed the orientation and submitted your certificate, you can move forward with creating your Person-Centered Plan (PCP).

What is a Person-Centered Plan (PCP)?

A Person-Centered Plan (PCP) is a document that outlines your individual goals, preferences, and the services you need to achieve those goals. It’s a critical part of the Self-Determination Program because it ensures that the services you receive are tailored to your unique needs and preferences.

What is an Independent Facilitator, and how can they help me?

An Independent Facilitator is someone who assists you in creating your Person-Centered Plan and navigating the Self-Determination Program. Are your services covered by the Regional Center?

Yes, the transition into the Self-Determination Program, including Independent Facilitation, is funded by the Regional Center. Once you transition into Self-Determination, ongoing Independent Facilitation services are paid for through your Self-Determination Program budget.

How long does it take to create a Person-Centered Plan?

The timeline for creating a Person-Centered Plan depends on your specific needs and goals. Typically, it takes a few weeks to complete the process, from gathering information to finalizing your plan. However, our team works with you to ensure that the plan is comprehensive and meets your unique needs.

What should I bring to the PCP interview?

At least 24 hours before the PCP interview, you should provide us with any relevant documents that will help us create a more complete plan, such as your IPP, IEP (if applicable), and any diagnostic evaluations. Also, once the interview is scheduled, you may complete the PCP Interview form.

What’s the next step after completing the orientation?

Once you’ve completed the Self-Determination Program Orientation, you’ll need to submit your certificate to us and then proceed with signing the required agreements and scheduling your PCP interview meeting. From there, we’ll work with you to create your Person-Centered Plan.

How often should I update my Person-Centered Plan?

Your Person-Centered Plan should be reviewed and updated regularly, especially if your goals or circumstances change. We recommend meeting with your Independent Facilitator at least once a year to ensure that your plan continues to reflect your evolving needs.
